RN Navigator
Job Purpose: The Nurse Navigator provides comprehensive diabetes education, care coordination, and chronic disease management for patients with diabetes. This role emphasizes patient empowerment, clinical support, patient triage, coordinating care based on patient needs, and guiding individuals to the correct level of care for their chief complaints while maintaining continuity of care. The Nurse Navigator will collaborate with providers, medical staff, and external healthcare organizations to facilitate seamless patient experiences and
improve health outcome.
Education: Graduation from an accredited nursing program. The employee is trained and prepared to care for the age-specific or specialty specific group served in their department.
Licensure/Certification/Registration: Possess BLS, ACLS, Registered Nurse New York State License.
Experience: Minimum of 2-3 years in a clinical nursing role.
Preferred: Leadership/supervisory experience or training, preferably in primary care, internal medicine, or triage settings; Bachelor's in Nursing
