Care Navigator

The Family Counseling Center of Fulton County (TFCC) is looking for a Care Navigator to support client engagement, streamline access to care, and ensure seamless service coordination. What You’ll Do:
- Guide clients through our services and match them with the care that best meets their needs
- Collaborate with clinical and intake teams to assess client needs and determine eligibility
- Coordinate internal and external referrals, ensuring timely and appropriate service connections
- Educate clients and families on available resources and assist with appointment scheduling
- Maintain accurate records in the electronic health record (EHR) system
- Develop partnerships with community organizations and follow up to ensure continuity of care
Qualifications:
- Bachelor’s degree in Social Work, Human Services, Public Health, or related field (Master’s preferred)
- 1–2 years of experience in healthcare, behavioral health, or social services
- Knowledge of NYS Medicaid, Health Home programs, and community resources
- Exceptional interpersonal and communication skills
- Commitment to person-centered, trauma-informed, and recovery-oriented care
- Strong organizational, problem-solving, and documentation skills
- Ability to work independently and collaboratively within a team
- Must pass a Medicaid exclusion screening and background check
