Community Service Provider

As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.

Responsibilities:

  • Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
  • Work with family members to identify goals and objectives of treatment
  • Provide services based on the needs as identified in the Treatment Plan
  • Coordinate with the child and caregiver on the scheduling and provision of services
  • Participate in relevant treatment meetings and attend all required trainings
  • Complete all documentation

Requirements and Education:

  • Bachelor's degree in a related field and at least two years of related experience OR
  • Master's degree in Social Work, counseling, psych, or a related human service and at least one year of related experience
  • Valid NYS Driver's License required
